Job summary

RHA Health Services, LLC is seeking a CAP Nurse Manager to oversee assessments, care planning, and service coordination for participants with intellectual and developmental disabilities and mental health needs in the New Hanover & Brunswick area. The role emphasizes high-quality, person-centered care and adherence to Medicaid guidelines within a multidisciplinary team.

The RN must coordinate services, monitor plans, and support families through counseling while ensuring accurate documentation

Qualifications

• Active NC RN license with 2- or 4-year degree required.
• Experience in case management and Medicaid programs preferred.
• Ability to educate families about waiver services and resources.
• Strong communication and multidisciplinary teamwork skills.

Responsibilities

• Assess beneficiaries and families to evaluate medical, psychological, behavioral, and social needs.
• Develop and update person-centered care plans at least every 12 months.
• Coordinate services such as personal care, waiver supplies, and home modifications.
• Monitor services via calls and home visits, document progress.
• Provide counseling and support to families.
• Ensure documentation compliance with Medicaid guidelines and CAP regulations.
• Participate in training and maintain up-to-date records in CAP Business system.
