Job summary

AdvantageCare Rehabilitation is seeking a licensed Physical Therapist for Centerburg Pointe in Centerburg, OH to deliver skilled rehabilitation on a PRN basis. You’ll conduct evaluations, design individualized plans, and guide residents through mobility, balance, and safety activities in a skilled nursing setting.

Qualifications

• CAPTE-accredited Physical Therapy program required.
• Current Ohio Physical Therapist license or eligibility to practice in Ohio (PT Compact privilege).
• Strong clinical reasoning, communication, and interpersonal skills.

Responsibilities

• Perform comprehensive PT evaluations of mobility, strength, balance and rehab needs.
• Develop individualized, evidence-based treatment plans with rehabilitation goals.
• Provide therapeutic exercises, gait, balance, transfers, and functional mobility interventions.
