Job summary

AdvantageCare Rehabilitation in Centerburg, OH is seeking a PRN Physical Therapist Assistant to provide skilled interventions under a licensed PT. This flexible role supports mobility, balance, gait, and functional independence for residents in a skilled nursing setting.

You'll work with a supportive team, remain closely supervised, and contribute to individualized treatment plans aligned with facility goals and patient needs.

Qualifications

• Graduation from a CAPTE-accredited Physical Therapist Assistant program.
• Current Ohio PTA license or eligibility to practice in Ohio prior to employment, including an applicable PT Compact privilege.
• Strong communication, interpersonal, and patient care skills.
• Ability to work collaboratively with a supervising Physical Therapist and interdisciplinary team.
• Commitment to delivering individualized, high-quality rehabilitation services.
• Experience in skilled nursing, geriatrics, or post-acute rehabilitation is a plus but not required.

Responsibilities

• Implement individualized physical therapy treatment plans established by the supervising Physical Therapist.
• Provide therapeutic exercises, gait training, balance activities, transfer training, and functional mobility interventions.
• Help residents improve strength, endurance, coordination, and independence with daily mobility.
• Monitor residents’ responses to treatment and communicate progress, concerns, or changes in condition to the supervising Physical Therapist.
• Provide patient and family education within the PTA scope of practice.
• Collaborate with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, nursing staff, and other interdisciplinary team members.
• Maintain accurate and timely clinical documentation in accordance with professional and regulatory standards.
• Assist with safe patient transfers, mobility training, and appropriate use of assistive devices.
• Follow Ohio PTA practice requirements, facility policies, and established safety protocols.
• Contribute to a positive, supportive, patient-centered rehabilitation environment.
