Job summary

Concept Rehab, Inc. is seeking PRN Physical Therapists (PTs) to provide skilled therapy across multiple Columbus-area SNFs in Ohio.

You will evaluate residents, develop individualized treatment plans, and collaborate with a wide interdisciplinary team to improve mobility and independence. Qualifications

Licensed Physical Therapist in Ohio
Strong communication and clinical decision-making
Compassionate, patient-centered care

Responsibilities

Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services to improve mobility, strength, function
Collaborate with physicians, nursing, families, and interdisciplinary team
Supervise PTAs as required
Participate in care planning and discharge planning
Recommend appropriate equipment and interventions to support patient outcomes
Complete timely, accurate documentation and billing
Educate patients, families, and facility staff on maintenance and restorative programs
