Job summary

AdvantageCare Rehabilitation is seeking a licensed Physical Therapist for Centerburg Pointe in Centerburg, OH to deliver skilled rehabilitation on a PRN basis. You’ll conduct evaluations, design individualized plans, and guide residents through mobility, balance, and safety activities in a skilled nursing setting.

Qualifications

• CAPTE-accredited Physical Therapy program required.
• Current Ohio Physical Therapist license or eligibility to practice in Ohio (PT Compact privilege).

What You'll Do

As a PRN Physical Therapist, you'll provide skilled rehabilitation services to residents in a skilled nursing and long-term care environment.

Your responsibilities will include:

• Conduct comprehensive physical therapy evaluations to assess mobility, strength, balance, functional limitations, and rehabilitation needs.

• Develop individualized, evidence-based treatment plans that support each resident's rehabilitation goals.

• Provide therapeutic exercises, gait training, balance activities, transfer training, and functional mobility interventions.

• Help residents improve mobility, restore function, reduce fall risk, and achieve greater independence.

• Monitor patient progress and adjust treatment plans based on clinical findings and patient response.

• Collaborate with Physical Therapist Assistants, Occupational Therapists, Speech-Language Pathologists, nursing staff, and other interdisciplinary team members.

• Educate residents and families on safe mobility, therapeutic exercises, fall prevention, and strategies to support continued progress.

• Maintain accurate and timely clinical documentation in accordance with professional and regulatory standards.

• Participate in care planning, progress reviews, and discharge recommendations as appropriate.

• Contribute to a positive, supportive, patient-centered rehabilitation environment.

Who We're Looking For

We're interested in connecting with Physical Therapists who bring compassion, clinical expertise, and a genuine commitment to helping others.

Required Qualifications:

• Graduate of a CAPTE-accredited Physical Therapy program.

• Current Ohio Physical Therapist license or eligibility to practice in Ohio prior to employment, including an applicable PT Compact privilege.

• Strong clinical reasoning, communication, and interpersonal skills.

• Ability to work independently while collaborating with an interdisciplinary team.

• Commitment to delivering individualized, high-quality rehabilitation services.

Experience in skilled nursing, geriatrics, or post-acute rehabilitation is a plus, but not required.

Experienced Physical Therapists and New Graduates Are Welcome to Apply!

Please note: This position requires Physical Therapist education and appropriate licensure. Physical Therapist Assistant (PTA), Rehabilitation Aide, or Therapy Technician credentials do not meet the qualifications for this PT position.
