Job summary

AdvantageCare Rehabilitation seeks a Physical Therapist to join our Centerburg, OH team on a PRN basis. You will evaluate and treat patients, develop personalized plans, and collaborate with physicians, nurses, and therapists to achieve optimal outcomes.

Qualifications

• Graduate of an APTA- or CAPT-accredited PT program.
• Hold a valid State Physical Therapy License.

Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, functional limitations, and mobility.
• Develop individualized treatment plans based on assessment and patient goals.
• Utilize therapeutic techniques including exercises, manual therapy, and modalities such as ultrasound, electrical stimulation, and heat/cold therapy.
• Maintain accurate documentation of evaluations, plans, progress, and discharge summaries.
• Communicate with healthcare professionals, patients, and families about plans and progress.
• Educate patients and families on the therapeutic process and home exercise programs.
