Job summary

Concept Rehab, Inc. is seeking a PRN Physical Therapist to provide exceptional care in a skilled nursing setting in Bowerston, OH. This flexible role allows you to work on your own schedule while making a meaningful impact on patients’ mobility and function.

You will evaluate patients, develop and monitor treatment plans, and collaborate with the care team to ensure high-quality patient care. Ohio licensure and PT degree are required, with SNF experience preferred but not required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Ohio PT license.
• Strong clinical reasoning, documentation, and communication skills.
• SNF experience preferred but not required.

Responsibilities

• Evaluate and treat patients to restore, maintain, and improve physical function and mobility.
• Develop individualized treatment plans and track progress according to facility standards.
• Collaborate with the interdisciplinary team to ensure high-quality patient care.
