Qualifications

• Valid, unrestricted license to practice Physical Therapy in the state.
• Educational requirements for licensure as required by the state.
• Valid driver’s license in the state of employment.
• Knowledge of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care.

Responsibilities

• Provide Physical Therapy services in a skilled nursing facility (SNF) setting.
• Collaborate with an interdisciplinary team to develop and implement patient-centered care plans.
