Responsibilities

• Screen and evaluate patients with cognitive and/or ADL impairments and develop a plan of care.
• Utilize standardized assessments and evidence-based practices to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment within regulatory and clinical standards.
• Document progress, adjust treatments, and communicate with the team regarding patient status.
• Prepare comprehensive discharge summaries per clinical requirements.
