Comprehensive Rehab Consultant in Palm Springs, CA is seeking a Part-time PM&R clinician to provide specialized Physiatry services in SNF settings. This W2 role emphasizes autonomy, collaboration with interdisciplinary teams, and delivering high-quality patient care during daytime hours.

The ideal candidate holds NP/PA license in California, has post-acute experience, and is comfortable working 8 hours around 9 AM–4 PM on weekdays.

Qualifications

• Nurse Practitioner/APRN/PA license valid in California.
• Post-acute/SNF experience preferred.
• Ability to work autonomously and deliver patient-centered care.

Responsibilities

• Provide specialized PM& R/Physiatry services as a rehab consultant.
• Represent CRC with clinical leadership and collaboration with the care team.
• Collaborate with PCPs to manage SNF patient conditions and improve function.
• Work with PT/OT/SLP teams to enhance quality of life and track progress.
• Attend multidisciplinary meetings and contribute to culture-building initiatives.
