Comprehensive Rehab Consultants seeks a PM&R Clinician (NP/PA) in Modesto, CA for a 16-hour, part-time W2 role. You will deliver PM&R/Physiatry services at a designated SNF or post-acute rehab center, collaborating with the therapy and medical teams to optimize patient outcomes.

Ideal candidates hold NP/PA licenses in California, have post-acute/SNF experience, and can work autonomously while maintaining compassionate patient care.

Qualifications

• Nurse Practitioner/APRN/PA license in the U.S.
• California license to practice
• Post-acute/SNF experience preferred
• Ability to work autonomously with strong patient focus

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant at a designated SNF or post-acute rehab center
• Represent CRC with clinical leadership within the facility and collaborate with the interdisciplinary team
• Coordinate with PCP teams for management of conditions common in SNFs
• Work with PT/OT/SLP teams to improve patient function and quality of life
• Follow therapy patients to track progress and assist in documentation
• Participate in multidisciplinary meetings and bridge therapy with internal medicine
• Engage in culture building and ongoing education activities
