Qualifications

• SNF/post-acute clinical experience preferred.
• Ability to function autonomously in a collaborative setting.
• Strong interpersonal skills and empathy toward geriatric patients.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNFs.
• Lead the facility clinically and collaborate with the care team.
• Coordinate with PCPs for management of common SNF conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Track patient progress and therapy barriers.
• Bridge therapy and internal medicine within the SNF team.
• Participate in multidisciplinary meetings and local education.
