Job summary

Comprehensive Rehab Consultants is seeking a Part-time PM&R Clinician to provide specialized Physiatry services at a designated SNF or post-acute rehab center in Palm Springs, CA. The 16-hour weekly W2 role offers autonomy within a supportive team and flexible scheduling within weekday hours.

You will collaborate with therapy teams and PCPs to optimize rehabilitation outcomes for geriatric patients, with a focus on functional independence and high-quality care.

Qualifications

Nurse Practitioner/APRN/PA license required.
California license required or eligible to practice in CA.
Ability to work in the U.S. without sponsorship.
Post-acute/SNF experience preferred.
Strong patient care and communication skills.

Responsibilities

Provide PM&R/Physiatry services as a rehab consultant in SNF.
Lead and collaborate with interdisciplinary care teams.
Coordinate with PCPs to manage post-acute conditions.
Support therapy teams to improve patient function and outcomes.
Monitor progress and address therapy barriers.
Engage in multidisciplinary meetings and culture-building activities.
