Qualifications

Active RN license in good standing (OH or Compact)
Minimum 1 year in long-term care
Reliable transportation to assigned facilities
Consistent attendance and punctuality
Able to push, pull, and lift at least 50 pounds
Legally eligible to work in the United States
Willing to complete background and drug screening

Responsibilities

Record patients’ medical information and vital signs in EMR Matrix
Assess resident health status and identify changes requiring nursing intervention
Administer medications and treatments per physician orders
Develop and evaluate individualized resident care plans
Coordinate care with physicians and interdisciplinary team
Provide clinical oversight to nursing and caregiving staff
Document assessments, interventions, and outcomes accurately
Maintain infection control standards and regulatory compliance
Communicate concerns to supervisor and agency
Other duties as assigned
