Qualifications

Responsibilities

• Record patient data and vital signs in EMR Matrix.
• Administer meds and treatments per orders and policies.
• Monitor health status and report changes promptly.
• Provide direct nursing care and support within scope.
• Assist with assessments and care plan implementation.
• Document meds, treatments, observations, and responses.
• Collaborate with CNAs, physicians, and staff.
• Communicate with residents, families, and providers.
• Maintain infection control and regulatory compliance.
• Escalate concerns to supervisor promptly.
• Other duties as assigned.

Responsibilities

An LPN picking up shifts through Elite Medical Staffing at a long-term care facility may be expected to:

• Utilize EMR Matrix to record patients’ medical information and vital signs
• Administer medications and treatments in accordance with physician orders and facility policies
• Monitor resident health status and promptly report significant changes in condition
• Provide direct nursing care and clinical support within scope of practice
• Assist with resident assessments and implementation of individualized care plans
• Accurately document medications, treatments, observations, and resident responses
• Collaborate with CNAs, medication aides, physicians, and other healthcare team members
• Communicate effectively with residents, families, and healthcare providers regarding resident needs
• Maintain compliance with facility policies, infection control standards, and regulatory requirements
• Communicate concerns or issues to onsite supervisor and agency
• Other duties as assigned

Qualifications

• Minimum 1 year of experience working under an active LPN license
• Minimum 1 year of experience working in long-term care
• Must have reliable transportation to assigned facilities
• Must maintain consistent attendance and punctualityAble to push, pull, and lift at least 50 pounds
• Able to be standing and walking with frequent bending, reaching, and lifting with proper body mechanics
• Must be legally authorized to work in the United States
• Willing to complete background and drug screening in accordance with applicable laws and regulations

Credentials Required

• Active and unencumbered OHLPN License or Compact LPN License
• Government-Issued Photo ID or Passport
• CPR/BLS
• Tuberculosis (TB) Test
• Flu Vaccine (during Flu season)
• Covid-19 Vaccine (religious/medical exemptions accepted)
• Resume
• Additional credentials may be required based on facility placement
