Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to provide in-home and community-based PT services across New Jersey. You will evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions to maximize independence and community participation.

Requirements include a NJ PT license, CAPTE-accredited education, CPR/BLS, and a clean background check.

Qualifications

• Current New Jersey license as a Physical Therapist.
• Graduate of a CAPTE-accredited PT program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ requirements.
• Reliable transportation and valid NJ driver’s license.

Responsibilities

• Perform in-home and community-based PT evaluations and develop individualized treatment plans.
• Deliver one-on-one therapy in home and community settings.
• Document sessions and outcomes per standards and timelines.
• Collaborate with coordinators, families, and the inter-disciplinary team to ensure continuity of care.
• Provide caregiver training and home exercise programs to reinforce goals.
• Adhere to program policies, privacy rules, and mandated trainings.
• Maintain billing records consistent with applicable rates and authorizations.
