Job summary

Beyond Home Health Care in New Jersey is seeking a licensed Physical Therapist to provide in-home and community-based PT services under NJ DDD guidelines. This per diem/contract role offers flexible scheduling and a private caseload in participants' homes.

You will evaluate mobility, strength, balance, gait and functional movement, develop ISP-aligned plans, document progress, and collaborate with families and the interdisciplinary team to maximize independence and community participation.

Qualifications

• NJ license as Physical Therapist (PT) issued by NJ State Board of Physical Therapy Examiners.
• CAPTE-accredited PT program (DPT or equivalent).
• Minimum 1 year clinical experience preferred; new grads considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.
• Ability to pass DDD background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one PT services in home and community settings.
• Document services per DDD standards with session notes, progress notes, and outcomes.
• Collaborate with Support Coordinators, families, caregivers and the team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies and HIPAA confidentiality standards.
• Maintain accurate billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle’s/Stephen Komninos’ Laws).
