Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to provide in-home and community-based PT services to adults with developmental disabilities across New Jersey. The PT will evaluate mobility, strength, balance, gait, and functional movement and deliver person-centered interventions to maximize independence and community participation.

Qualifications

• NJ licensed Physical Therapist (PT)
• CAPTE-accredited PT program (DPT or equivalent)
• Current CPR/BLS certification
• Clean criminal background check and fingerprinting per NJ DDD requirements
• Reliable transportation and NJ driver’s license

Responsibilities

• Conduct comprehensive home and community-based PT evaluations.
• Develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy in home/community settings.
• Document services per DDD standards and timelines.
• Collaborate with coordinators, families, and interdisciplinary team.
• Provide caregiver training and home exercise programs.
• Maintain compliance with DDD policies and HIPAA requirements.
• Complete required DDD trainings.
