Job summary

Beyond Home Health Care seeks a licensed Physical Therapist to deliver in-home and community-based PT services to adults with developmental disabilities. You will evaluate mobility, strength, balance, gait, and functional movement, and provide person-centered interventions to maximize independence and participation.

This per diem/contract role covers statewide New Jersey with flexible scheduling. Compensation ranges from $76 to $82 per hour, with mileage considerations and strong clinical

Qualifications

• Current New Jersey license as a Physical Therapist (PT).
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver's license, and current auto insurance.
• Ability to pass DDD-required background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP goals.
• Deliver one-on-one therapy services in home and community settings.
• Document services in compliance with DDD standards, including progress notes and outcomes.
• Collaborate with Support Coordinators, families, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies and HIPAA confidentiality standards.
• Maintain billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle's Law, etc.).
