Job summary

Beyond Home Health Care in New Jersey seeks a licensed Physical Therapist to provide in-home and community-based PT services to adults with developmental disabilities. This per diem/contract role offers flexible scheduling statewide, with compensation of $76–$82 per hour.

The PT will evaluate mobility, design ISP-aligned treatment plans, document progress, train caregivers, and collaborate with families and the interdisciplinary team, all while complying with NJ DDD policies and HIPAA.

Qualifications

• Current NJ license as Physical Therapist (PT) issued by the NJ State Board.
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.
• Ability to pass DDD background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in the participant's home and community settings.
• Document services in accordance with DDD standards, including session notes, progress notes, and outcome measures.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
• Adhere to DDD policies, Danielle's Law, Stephen Komninos' Law, and HIPAA confidentiality standards.
• Maintain accurate billing records consistent with DDD Fee-for-Service rates and authorizations.
• Complete required DDD-mandated trainings.
