Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to deliver in-home and community-based PT services to adults with developmental disabilities in New Jersey. This per diem/contract role offers flexible scheduling and compensation of $76–$82 per hour.

The PT will evaluate mobility, balance, gait, and functional movement, developing individualized plans to maximize independence and community participation.

Qualifications

• Current New Jersey license as a Physical Therapist (PT).
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver's license, and current auto insurance.
• Ability to pass DDD-required background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy services in home and community settings.
• Document services per DDD standards, including session and progress notes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals.
• Maintain compliance with DDD policies and HIPAA confidentiality.
• Maintain billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings.
