Job summary

Beyond Home Health Care, a NJ DDD-Approved provider, seeks a licensed Physical Therapist to deliver in-home and community-based PT services to adults with developmental disabilities. You will evaluate mobility, balance, and functional movement, and implement person-centered interventions to maximize independence.

The role involves one-on-one sessions in homes, collaboration with families and coordinators, and adherence to DDD policies.

Qualifications

• Current New Jersey license as a Physical Therapist (PT) issued by the NJ State Board of Physical Therapy Examiners.
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• CPR/BLS certification is current and valid.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation with valid NJ drivers license and auto insurance.

Responsibilities

• Conduct comprehensive in-home evaluations and develop individualized treatment plans aligned with ISP goals.
• Deliver one-on-one PT services in participant homes and community settings.
• Document services per DDD standards: session notes, progress notes, outcomes.
• Collaborate with Support Coordinators, families, caregivers, and the inter-disciplinary team.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to DDD policies, Danielle's Law & HIPAA confidentiality standards.
• Maintain billing records consistent with DDD Fee-for-Service rates and authorizations.
• Complete required DDD trainings and certifications.
