Job summary

The Care Team Hospice is seeking compassionate On-Call RNs to provide after-hours and weekend support to hospice patients and families. You’ll respond to patient needs, complete visits when necessary, manage symptoms, and support families while remaining at home between on-call responses.

This role offers scheduling flexibility including Weekdays, Weekends, and 7 On/7 Off rotations, with independence and a clinical team guiding you.

Qualifications

• Active and unrestricted Registered Nurse license in the state of practice.
• Minimum of one year of nursing experience.
• Hospice, home health, oncology, palliative care, or critical care experience preferred.
• Ability to work independently and respond to changing patient needs.
• Valid driver’s license, reliable transportation, and current automobile insurance required.

Responsibilities

• Respond to after-hours patient and family needs with nursing assessments, symptom management, and clinical interventions.
• Complete scheduled and urgent patient visits, addressing changes in condition and providing end-of-life care.
• Educate and support patients and caregivers on symptom management, medications, and end-of-life transitions.
• Collaborate with physicians, caregivers, and the interdisciplinary team to ensure continuity of care.
• Complete accurate and timely clinical documentation within the EMR.
