Job summary

The Care Team Hospice is seeking an On-Call Registered Nurse (RN) to provide after-hours support to hospice patients and families, while working from home between visits. This role emphasizes independent clinical decision-making, symptom management, and compassionate end-of-life care within an experienced interdisciplinary team.

Weekday on-call coverage is Monday–Thursday 4:30 PM–8:00 AM, with a mix of scheduled patient visits and on-call responsibilities.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of one year of nursing experience; hospice/home health preferred.
• Strong clinical assessment, symptom management, and critical-thinking skills.
• Ability to work independently and make confident clinical decisions.
• Excellent communication and patient-centered approach.
• Experience with EMR; Homecare Homebase (HCHB) preferred.

Responsibilities

• Respond to after-hours patient and caregiver needs, providing timely nursing assessments and interventions.
• Complete scheduled and urgent patient visits with end-of-life care emphasis.
• Support patients and families through symptom management and education.
• Coordinate with physicians, pharmacies, and interdisciplinary team for continuity of care.
• Document accurately in the EMR in a timely manner.
