Job summary

The Care Team Home Health & Hospice is seeking an On-Call Registered Nurse to join a compassionate interdisciplinary team serving hospice patients in their homes and facilities. The role offers flexibility with weekday or weekend on-call shifts and mostly remote work when not in the field.

Responsibilities include skilled nursing assessments, symptom management, crisis intervention, and coordination with physicians, caregivers, and the care team.

Qualifications

• Active RN license in the state of practice.
• At least 1 year of nursing experience; hospice/home health preferred.
• Strong assessment, symptom management, and decision-making skills.
• Experience with EMR systems, especially HCHB.

Responsibilities

• Provide after-hours and weekend nursing assessments and interventions.
• Perform on-call visits to assess condition changes and adjust care plans.
• Coordinate care with physicians, caregivers, and interdisciplinary team.
• Educate patients, families, and caregivers on medications and symptom management.
• Facilitate admissions, end-of-life processes, and urgent clinical situations per policy.
• Document assessments and care coordination promptly in the EMR.
• Deliver compassionate, patient-centered care supporting patients and families.
