Hayward Gardens Post Acute is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team in Hayward, CA. We offer full-time, part-time, and PRN opportunities with a pay range of $65.00–$75.00 per hour (DOE).

Qualifications

• Current California Occupational Therapist (OTR) license in good standing.
• Graduate of an accredited Occupational Therapy program.
• CPR/BLS certification as per facility policy.
• Experience in skilled nursing or post-acute rehab preferred; new grads okay.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Evaluate residents and determine OT needs under physician orders.
• Develop and modify treatment plans to maximize ADLs and mobility.
• Supervise COTAs and rehab staff according to regulations.
• Collaborate with physicians, nursing, and families on care plans.
• Document timely and compliant treatment records per policy.
• Educate residents and staff on home exercises and safety.
• Assist with equipment orders to improve independence.
