Job summary

Pleasant Hill Post Acute in Pleasant Hill, CA is hiring an Occupational Therapist (OT) to join our rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities for OTs passionate about helping patients regain independence and improve function.

Qualified candidates will hold a California OT license, work within an interdisciplinary team, and provide patient-centered care with strong documentation and communication skills. Compensation ranges are based on experience.

Qualifications

• Current California Occupational Therapist (OT) license in good standing.
• Strong clinical, communication, and organizational skills.
• Ability to work effectively as part of an interdisciplinary team.
• New graduates are welcome to apply.
• Skilled nursing or post-acute rehabilitation experience preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on their functional needs and goals.
• Provide skilled occupational therapy services to help patients improve independence with activities of daily living.
• Help patients maximize functional abilities and improve their overall quality of life.
• Monitor progress and adjust treatment plans as appropriate.
• Collaborate with physicians, nursing staff, physical therapists, speech therapists, and other members of the interdisciplinary team.
• Communicate effectively with patients, families, and caregivers regarding treatment goals and progress.
• Complete accurate, timely, and compliant clinical documentation.
• Provide compassionate, patient-centered care while maintaining professional standards.
