Job summary

Hayward Gardens Post Acute in Hayward, CA is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team. We offer full-time, part-time, or PRN options with competitive pay and a supportive environment.

The OTR will evaluate residents, develop individualized plans, and supervise staff while working with physicians, families, and the care team to maximize independence and quality of life in a skilled nursing/post-acute setting.

Qualifications

• Must be a licensed OTR in California.
• Graduate of an accredited OT program is required.
• CPR/BLS certification is preferred or required by policy.

Responsibilities

• Evaluate residents and determine OT needs and rehab potential.
• Develop and modify individualized treatment plans to maximize independence.
• Supervise COTAs and rehabilitative staff per state rules.
• Collaborate with physicians and the interdisciplinary team.
• Document timely and accurately to meet regulatory standards.
• Educate staff and families on home programs and safety.
