Job summary

Hayward Gardens Post Acute | Hayward, CA

Pay: $65.00-$75.00 per hour (DOE)

Hayward Gardens Post Acute is seeking a compassionate, motivated, and dedicated Registered Occupational Therapist (OTR) to join our rehabilitation team. We are hiring Full-Time, Part-Time, and PRN therapists who are passionate about helping residents achieve their highest level of independence and quality of life in a skilled nursing and post-acute setting.

Position Summary

The Staff Occupational Therapist evaluates and treats residents, develops individualized treatment plans, collaborates with the interdisciplinary care team, communicates with physicians and families, and maintains accurate clinical documentation. This position also provides education and training to facility staff and supervises Occupational Therapy Assistants (COTAs), aides, and students.

Essential Responsibilities
  • Evaluate residents to determine occupational therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and modify individualized treatment plans to maximize independence in activities of daily living (ADLs), mobility, strength, balance, endurance, and overall function.
  • Supervise Occupational Therapy Assistants (COTAs) in accordance with state practice regulations.
  • Assist the nursing department with training and oversight of Restorative Aides.
  • Collaborate with physicians, nursing staff, interdisciplinary team members, residents, and families regarding treatment progress, care planning, and discharge recommendations.
  • Participate in Resident Care Conferences, Rehabilitation Meetings, and Utilization Review meetings as needed.
  • Provide in-service education and training for facility staff.
  • Complete timely, accurate, and compliant documentation in accordance with facility, state, federal, and payer requirements.
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.
  • Educate residents, families, and caregivers on home exercise programs, safety techniques, adaptive equipment, fall prevention, and maintenance programs.
  • Recommend and facilitate ordering of adaptive equipment and durable medical equipment (DME) to improve resident independence.
  • Maintain confidentiality of resident and company information.
  • Report equipment issues to ensure proper maintenance and safe operation.
  • Participate in quality assurance and performance improvement initiatives.
  • Foster positive, professional relationships with residents, families, staff, and healthcare professionals.
Supervisory Responsibilities
  • Supervise Certified Occupational Therapy Assistants (COTAs), rehabilitation aides, and occupational therapy students.
Qualifications
  • Current California Occupational Therapist (OTR) license in good standing.
  • Graduate of an accredited Occupational Therapy program.
  • Current CPR/BLS certification (preferred or required per facility policy).
  • Experience in skilled nursing, post-acute care, or rehabilitation is preferred, but new graduates are encouraged to apply.
  • Strong communication, documentation, and organizational skills.Ability to work collaboratively in a fast-paced interdisciplinary environment.
