Job summary

Oakwood Gardens Care Center in Fresno, CA is seeking a PRN Occupational Therapist (OT) to join our team. The role offers competitive hourly pay and a full-time benefits package for a capable clinician serving residents in a post-acute setting.

The OT will evaluate residents, develop treatment plans, provide direct therapy, and supervise COTAs while coordinating with the interdisciplinary team. CA license and EMR familiarity are required or preferred.

Qualifications

• Licensed OT in California in good standing.
• Experience with EMR systems such as Casamba or PointClickCare is a plus.
• SNF or post-acute rehab experience preferred but not required.

Responsibilities

• Evaluate residents to determine functional abilities and therapy needs.
• Develop and implement individualized treatment plans.
• Provide direct therapy services per physician orders.
• Supervise COTAs and collaborate with interdisciplinary team.
• Document progress in compliance with regulatory standards.
• Participate in discharge planning and caregiver education.
• Contribute to in-service training and quality improvement initiatives.
