Job summary

Powerback is seeking a Physical Therapist for a PRN position (10 hrs/week) in West Hartford, CT. The role focuses on outpatient rehabilitation delivered in home and clinic settings for older adults, with a flexible schedule that supports work-life balance.

The Physical Therapist will assess patients, develop and implement therapy programs, and collaborate with the rehab team to optimize patient outcomes. Valid PT license in CT and reliable transportation are required.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy (or equivalent with related degree and experience).
• Driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in home or facility settings for older adults.
• Assess, plan, and implement physical therapy programs; document and communicate progress.
• Supervise PTAs, temporary licensees, students and support personnel as allowed by state licensure.
• Collaborate with the rehabilitation team to select the most appropriate therapeutic regimens.
• Promote safety, patient-centered care, and timely care coordination.
