Job summary

FeldCare Connects is seeking a passionate Physi​cal Therapist to provide high-quality home healthcare for patients of all ages in Chico and surrounding areas. This Independent Contractor role offers flexible scheduling and control over your coverage, with the ability to determine patient load and travel.

Qualifications

• Ability and enthusiasm in working with all ages.
• Current Physical Therapist license issued by the state Board of Physical Therapy.
• License must be current, active, and unrestricted; conditional/provisional/restricted licenses not accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver one-on-one home healthcare treatments to patients of all ages.
• Observe, record, and report patient responses and changes to the plan of care.
• Coordinate with patients and families to implement the care plan.
• Provide documentation via an intuitive Electronic Medical Record system.
• Utilize Cliniconnects to accept or decline patients quickly.
