Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier care in home health for adults of all ages. You will assess patient needs, determine treatment plans, and execute interventions to promote movement and reduce pain.

This position is for an Independent Contractor serving the CA area. The role requires a Physical Therapist license and graduation from an approved PT curriculum.

Qualifications

• Ability and enthusiasm in working with all ages
• Physical Therapist license and registration by the state
• Completion of a Physical Therapist curriculum approved by The American Physical Therapy Association or equivalent

Responsibilities

• Assess and evaluate patient care needs to promote movement, pain reduction, function restoration and disability prevention.
• Determine a treatment plan based on assessment and evaluation.
• Execute interventions to improve and enhance patient well-being and monitor progress.
