Job summary

FeldCare Connects is seeking a motivated Physical Therapist to provide quality home healthcare to patients of all ages as an Independent Contractor. The role covers Newbury Park and surrounding areas, using Cliniconnects for patient access and documentation.

You will deliver one-on-one treatment, observe patient progress, and tailor the plan of care while maintaining flexible scheduling and autonomy in your practice.

Qualifications

• Current Physical Therapist license must be active and unrestricted.
• Accredited Physical Therapy program completion is required.
• Reliable personal transportation and a smartphone are needed.
• Bilingual ability is a plus.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report patient response to treatment.
• Determine and implement the plan of care for each patient.
• Use Cliniconnects app to accept or decline patients in moments.
