Job summary

OneWellHealth Care seeks a dedicated Direct Support Professional (DSP) to assist individuals with IDD in Lafayette Hill, PA. This field-based role emphasizes person-centered care, daily living activities, and community integration.

The DSP will follow individualized care plans, monitor health and safety, and document services in EVV and other systems. A high school diploma and valid driver’s license are typically required, with non-exempt status and flexible shifts.

Qualifications

• High school diploma or equivalent.
• Experience in home health or direct support for individuals with IDD preferred.
• Ability to travel to client homes and/or community sites as needed.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meals, mobility, and medication assistance as authorized.
• Implement behavior support strategies and document care activities per ISP/IPP and regulatory requirements.
• Support transportation, community participation, and scheduling flexibility while maintaining safety and privacy.
