Job summary

OneWell Health Care in Connecticut is seeking a dedicated Direct Support Professional (DSP) in Middletown to provide person-centered, community-based support to individuals with IDD and Autism. The DSP assists with daily living activities, skill development, health and safety monitoring, and community participation.

The role emphasizes dignity, independence, and inclusion, requires adherence to ISP and BSP plans, and may involve field-based work across CT with a flexible schedule and non-exempt

Qualifications

• High School Diploma or GED required.
• Minimum age 18 years.
• English proficiency required (spoken and written).
• Valid Driver's License with reliable transportation.

Responsibilities

• Provide person-centered support with ADLs and ISP/BSP implementation.
• Support community integration and independence while ensuring health and safety.
• Complete accurate documentation: daily notes, MARs, incident reports.
