Job summary
OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) in Winchester, CT. The DSP provides person-centered support to individuals with IDD and Autism, assisting with daily living activities, health monitoring, and community participation, while promoting dignity and independence in line with state standards.
The role requires adherence to ISP/BSP plans, documentation, and collaboration with interdisciplinary teams.
Qualifications
- This role requires experience or familiarity with IDD/Autism support services and ISP/BSP-based plans.
- Candidates should be able to document accurately and follow safety protocols.
- Experience with community-based services is preferred.
Responsibilities
- Provide person-centered support in daily living activities and community participation.
- Maintain ISPs and BSPs, monitor health and safety, and ensure dignity and independence.
- Assist with transportation, appointments, and documentation such as MARs and progress notes.
