Job summary

OneWell Health Care in Bath, PA is seeking a dedicated Caregiver / Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings.

This field-based role involves assisting with daily living, health monitoring, and community participation, with flexible scheduling and compliance with privacy and care plans. English proficiency required; bilingual a plus.

Qualifications

• High school diploma or equivalent.
• English proficiency required; bilingual a plus.
• Experience in caregiving or home health preferred.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home/community settings.
• Assist with personal care, daily living activities, meal prep, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Maintain structured routines, documentation, and ISP/IPP guidance with team.
• Support transportation to appointments and community outings; participate in trainings.
