Job summary

OneWell Health Care in Coatesville, PA is seeking a dedicated Caregiver/Direct Support Professional (DSP) to support individuals with IDD in home and community settings, focusing on daily living assistance and skill development.

The DSP will follow ISP/IPP care plans, document activities, and collaborate with an interdisciplinary team. English proficiency is required; a valid driver’s license may be needed; flexible scheduling including evenings and weekends.

Qualifications

High school diploma or equivalent.
English proficiency (spoken and written) is required.
Bilingual skills are a plus, depending on program and individual needs.
Ability to follow written care plans and documentation guidelines.
Ability to use electronic documentation and EVV systems.

Responsibilities

Provide hands-on, person-centered support to individuals with IDD.
Assist with daily living activities, mobility, meals, and medication assistance as authorized.
Implement ISP/IPP and behavior support strategies.
Monitor health, safety, and emotional well-being; support community participation.
Maintain accurate and timely documentation and participate in trainings.
