Job summary

OneWell Health Care is seeking a dedicated Caregiver/Direct Support Professional (DSP) in Tyler Hill, PA to provide compassionate, person-centered support in home and community settings.

The DSP will assist with daily living activities, health and safety monitoring, transportation, and documentation, while promoting independence and dignity for individuals with IDD and autism, under supervision.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving, home health, or direct support preferred.
• Ability to follow care plans and documentation guidelines.

Responsibilities

• Provide hands-on, person-centered support in home/community settings.
• Assist with daily living activities and health monitoring.
• Support meal preparation, mobility, and transportation to appointments.
