Job summary

OneWell Health Care in Southampton, PA is seeking a dedicated Caregiver/Direct Support Professional (DSP) to provide hands-on, person-centered support in home and community settings.

You will assist with daily living activities, document care plans, support health and safety, and promote independence while upholding dignity and inclusion for individuals with IDD. Flexible schedule and field-based work offered. Training provided; teamwork with interdisciplinary staff; ensure HIPAA compliance.

Qualifications

• High school diploma or equivalent.
• Preferred: caregiving or direct support experience.
• English proficiency required; bilingual a plus.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain structured routines and support transportation to appointments or community activities.
• Document notes accurately and timely; participate in trainings and team meetings.
