Job summary

Arizona Autism United (AZA United) is hiring a Field Trainer for Habilitation Services to support HCBS/DSS with matching, monitoring and training for habilitation, respite and attendant care providers in the East Valley (Tempe, Chandler, Gilbert, Mesa). The role involves home visits, remote coordination, and collaboration with the program director.

Requires 1+ year of home-based habilitation experience, GED, and reliable transportation.

Qualifications

• GED required; Associate's degree or equivalent years of related work experience preferred.
• At least 1 year of experience providing home-based habilitation for multiple children/youth with autism.
• Must have own transportation with proof of insurance and registration.
• Must pass fingerprint clearance check and tuberculosis screening.

Responsibilities

• Provide training and support for Habilitation, Respite & Attendant Care Providers.
• Conduct home visits to assist providers with hands-on training and guidance.
• Help families prepare for ISP meetings with DDD and attend if needed.
• Deliver remote support via phone, email or virtual meetings as needed.
• Follow up on compliance items and maintain documentation per guidelines.
