Job summary

Activate Care is seeking a community-focused RN to travel across Massachusetts, performing in-person health assessments for dually eligible members and documenting findings for care management handoff. This role emphasizes assessment accuracy and timely SOAP notes rather than ongoing care management.

The candidate should have MA RN licensure, 1+ year home health experience, strong communication, and ability to document in electronic platforms; travel may exceed 50% and influenza vaccination is

Qualifications

• Active RN license in good standing with MA.
• ASN required; BSN preferred.
• Valid driver’s license, vehicle, and insurance.
• Minimum 1 year clinical home health experience.
• Minimum 1 year assessments experience preferred.
• Experience with dual-eligible populations a plus.
• RAC-CT or similar certification preferred.
• Ability to document via secure electronic platforms.

Responsibilities

• Conduct in-home health assessments for MA members.
• Document findings and hand off to care management.
• Submit SOAP notes within 48 hours of assessment.
• Routinely travel to members' homes and community settings.
• Maintain regulatory, HIPAA, and NCQA standards.
• Complete required and ongoing trainings.
