Job summary

Activate Care is seeking a Registered Nurse to travel to members’ homes across Massachusetts to perform in-person health assessments for a managed care health plan. The role emphasizes accurate assessment, documentation, and hand-off to care management, rather than ongoing care management.

The candidate must be an RN with MA licensure, hold a valid driver’s license, and have home health experience. A BSN is preferred; RAC-CT or similar certifications are a plus.

Qualifications

• Active RN license in MA in good standing.
• 1 year of clinical experience in home health.
• Assessment experience preferred (1 year).
• Valid driver’s license and insured vehicle.

Responsibilities

• Travel to members' homes across Massachusetts to perform health assessments.
• Document assessments in the health plan’s platform and send SOAP notes.
• Maintain timely documentation within 48 hours of assessments.
• Maintain licensure and participate in required trainings.
