Job summary

Activate Care in Massachusetts seeks a field-based Registered Nurse to perform in-home health assessments for dually eligible members under a managed care plan. You will travel to members' homes across MA, completing timely assessments and documenting findings for handoff to care managers.

Active MA RN license required, Associate Degree in Nursing (BSN preferred). The role requires 50%+ travel, a valid driver’s license, and strong organizational skills with HIPAA and NCQA compliance for health

Qualifications

• Active Massachusetts RN license in good standing.
• Associate Degree in Nursing required; BSN preferred.
• 1+ years of home health experience.
• 1+ years of assessment experience preferred (RAC-CT or similar certification a plus).
• Experience with dual-eligible, Medicaid, or Medicare Managed Care populations preferred.
• Sound clinical judgment with strong organizational and decision-making skills.

Responsibilities

• Conduct in-person assessments in members' homes and community settings.
• Document assessment findings in GuidingCare and send SOAP notes to the care manager.
• Submit complete documentation within 48 hours of each assessment and address audit findings within two business days.
• Follow field-safety guidelines and escalate safety or clinical concerns as needed.
• Maintain compliance with HIPAA, NCQA, and regulatory requirements; complete ongoing training.
