Job summary

Activate Care is seeking a registered nurse to conduct in-home health assessments for Massachusetts members under a managed care plan. The role involves traveling to homes and community sites, completing standardized assessments, documenting findings, and handing off to care management teams.

This is assessment and documentation work, not ongoing clinical management. The position requires MA RN licensure, at least 1 year in home health, and comfort working independently.

Qualifications

• Active RN or higher licensure in MA.
• AS in Nursing required; BSN preferred.
• Valid driver’s license, vehicle and insurance.
• Minimum 1 year in a home health setting.
• Minimum 1 year of assessment experience.
• Prior dual-eligible/Medicaid/Medicare experience preferred.
• Assessment certification (e.g., RAC-CT) preferred.
• Ability to work independently in members’ homes.
• Ability to document in a secure electronic platform.

Responsibilities

• Engage with members in homes and community settings, mindful of cultural/linguistic needs.
• Perform timely assessments using UCA, FA, and Comprehensive Assessments.
• Document assessments in GuidingCare per workflows.
• Send SOAP notes to Care Manager after assessment.
• Submit accurate documentation within 48 hours of each assessment.
• Complete required and ongoing training for system access.
• Routinely travel to members’ homes and community settings.
• Report abuse or neglect as a mandated reporter.
• Follow safety and regulatory standards (HIPAA, NCQA).
• Maintain licensure and certifications.
