Optum Home & Community Care seeks an RN Field Case Manager in Georgetown County, SC to coordinate care in nursing homes and home settings. You will document thoroughly, collaborate with the care team, support telehealth, and assist with Medicare benefit determinations.

The role requires an RN license in SC, 3+ years of clinical experience, BLS, and strong computer skills. Travel between member locations daily is required, with a comprehensive benefits package.

Competenze

• RN license to practice in South Carolina (SC).
• Minimum of 3 years of clinical practice experience.
• BLS certification and ability to document accurately.
• Proficient in MS Office and EMR documentation.
• Willingness to travel between member locations daily.

Mansioni

• Ensure accurate and complete documentation in patient records.
• Collaborate with interdisciplinary care team to coordinate care.
