Job summary

Optum Home & Community Care in South Carolina is seeking a qualified RN to join our Senior Community Care team, delivering care across nursing homes, senior housing, and assisted living settings. You will support telehealth, coordinate care with the interdisciplinary team, and assist with Medicare benefits.

Qualifications

• Associate degree or higher in Nursing (RN)
• Active and unrestricted RN license in SC
• Certified in Basic Life Support
• 3+ years of clinical experience in practice
• Ability to complete physical requirements including lifting 30 lb and performing physical assessment
• Proficient in Microsoft Office products (Word, Excel, Teams)
• Strong communication and time-management skills
• Ability to travel between member locations daily

Responsibilities

• Ensure accurate and complete documentation
• Collaborate with the interdisciplinary care team
• Support telehealth visits in the field
• Assist with care coordination activities
• Assist with Medicare Part A, B, and D benefits
• Coordinate with APC and facility on patient condition changes
• Participate in facility-based meetings and family conferences
• Manage training, quality measures, and documentation accuracy
