Job summary

Optum is hiring an RN Field Clinician to service Cumberland, Harnett and Hoke Counties in NC. You will work with the Senior Community Care (SCC) team to deliver coordinated nursing care in facility and home settings.

The role emphasizes assessments, care coordination, and collaboration with an interdisciplinary team to improve patient outcomes. A sign-on bonus of $2,000 is offered for external candidates and a comprehensive benefits package is provided.

Qualifications

• Associate degree or higher in Nursing (RN)
• Active and unrestricted license in the state of residence or ability to obtain by start date
• Certified in Basic Life Support
• Ability to complete physical requirements, including moving a 30-pound bag and performing physical assessment
• Active and unrestricted driver’s license and access to reliable transportation for home visit assessments

Responsibilities

• Ensure accurate and complete patient assessments
• Communicate and collaborate with the interdisciplinary care team
• Assist the provider/team with care coordination activities
• Assist with Medicare Part A, Part B, and Part D benefit determinations
• Collaborate with APC and facility to identify and respond to changes in patient condition
• Assist with therapy coordination and facility-based meetings
• Schedule and participate in family conferences and team case presentations
• Coordinate training, tracking, and compliance with quality measures
• Assist in creating and conduct in-services for selected audiences
• Support documentation, medication reconciliation, nursing rounds, and charting
