Job summary

Optum, a part of UnitedHealth Group, seeks an experienced Field RN Case Manager to support residents in senior living communities in the Salem, OR area. You will coordinate care across facilities, families, and telehealth, ensuring accurate documentation and proactive care planning.

The role requires an OR RN license, Basic Life Support, 3+ years of clinical experience, and strong communication skills. Mon-Fri schedule; no on-call, weekends, or holidays. A $7,500 sign-on bonus is offered.

Qualifications

• Active and unrestricted RN license in OR.
• Associate degree or higher in Nursing (RN).
• Certified in Basic Life Support.
• 3+ years of clinical practice experience.
• Reliable transportation and daily travel between locations.
• Proficient in documenting medical information in EMR and paper records.
• Proficient in Word, Excel and Teams.

Responsibilities

• Ensure accurate and complete documentation.
• Collaborate with the interdisciplinary care team.
• Support telehealth visits in the field and set up telehealth with the APC.
• Assist with care coordination activities and benefit determination (Medicare Part A/B/D).
• Work with APC and facility to respond to patient condition changes.
• Coordinate therapy and participate in meetings.
• Schedule and participate in family conferences and team meetings.
• Coordinate training and ensure quality measures compliance.
• Prepare in-services for selected audiences.
• Assist with documentation, coding, medication reconciliation, and rounds.
• May write verbal orders in compliance with RN practice laws.
• Manage patients through acute events or chronic exacerbations.
• Assist in obtaining transfer documentation for SNF stays.
